This time period is marked by high decision-making and adjustment demands. More obvious physical symptoms cause defenses to break down. Patients are forced to confront reality and accompanying feelings of fear, grief, anger, sadness, and helplessness. Regressions commonly occur and any previous mental health problems are exacerbated.

Those who pursue a living transplant may confront feelings of guilt and powerlessness over the potential sacrifice of another. Those on the deceased kidney donation wait list may experience feelings of worthlessness and devaluation as one of many waiting to find out if they get to live.

The nearness of further decline brings up thoughts of mortality and intense feelings while asking more of patients, their support system, and nephrologists. Adapting to the multiple changes during this phase of illness is critical for survival.
